Welcome to the E-Sign On-Line™ Process at UCLA. In the next few web pages you will be asked to complete and submit 3 online forms in 3 steps:
- STEP 1: Entrance Personal Data Form
- STEP 2: Rights and Responsibilities Form
- STEP 3: Perkins Master Promissory Note (via E-Sign On-Line™ through the ACS website)
In order for the Perkins Loan you accepted on your eFAN to disburse, you are required to complete these forms. Carefully read and follow the instructions on each page before proceeding to the next one. DO NOT USE the back button. YOU MUST COMPLETE ALL 3 FORMS IN ONE SITTING, so it is important for you to have all required information close at hand.
Before you continue, please make certain you have available the following:

Your FAFSA PIN (If FAFSA PIN unavailable/lost, go to www.pin.ed.gov and request new/duplicate one.)
(Authentication through Experian Credit Service only acceptable when e-signing non-Perkins Loan documents.)
- Your UCLA Logon ID
- Home and work contact information for your spouse (if applicable)
- Home and work contact information for your parents/guardians
- Home and work contact information for 2 references
(The references can not be students, nor individuals living at the same address with you, your parents/guardians or each other. You may not use your parent/guardian as a reference. It is permitted to list as references siblings who are over 21 years of age and living on their own.)
- Computer capable of running either:
- Mozilla Firefox 2.0 or better
- Microsoft Internet Explorer 6.0 or better
- Apple Safari 2.0 or better
- Adobe Acrobat Reader 3.02 or better installed. (If you do not have Acrobat Reader, go to http://get.adobe.com/reader/ to install it and then return to this page.
- Ability to temporarily disable any pop-up blocking software.
If you have met the requirements listed above in preparation for completing the online forms, continue on to STEP 1 by first logging in to ISIS.
(If you prefer to not complete and submit your forms online but still want the Perkins Loan, please EXIT this webpage and come in person to the Loan Services Office, A227 Murphy Hall and request an approved printed set of Perkins Loan documents.)
